This page allows you to easily find any form that may be required as part of your study abroad experience. Unless otherwise noted, all forms are provided as PDF's, which require Adobe Acrobat Reader or any program capable of opening .pdf files.
Academic Advisor Form This form is required for all applicants. You are required to obtain your academic advisor's signature and submit this with your other application materials prior to the application deadline.
Application Fee Form This form is required from all applicants along with the $25.00 study abroad application fee. It must be submitted by the deadline of the program to which you are applying.
Cost Estimation Worksheet You may use this sheet to help estimate the total cost of your study abroad experience.
Course Accreditation Form This form is for use by students who are seeking academic approval for specific courses abroad or on National Student Exchange that are not on the list of pre-approved courses for the program you are attending.
Evaluators This list contains the names and contact information for departmental evaluators that can approve the courses you listed on the Course Evaluation Request Form (above).
International Student Identity Card application This application is for students who wish to apply for an ISIC card. Please read the instructions of the form carefully for eligibility requirements.
Pass/Fail Request Form Return a signed form to the Office of Study Abroad via fax or regular mail. Document must be signed, dated and mailed during the first two weeks of the class or within the first week of a summer class. Please be sure to save a copy for your files.
Petition for New Program This form is used if you wish to apply to study abroad on a program that is not already approved by the UConn Office of Study Abroad. Please contact the Office of Study Abroad before submitting this form.
Student Contract This contract is provided for reference purposes only. Students accepted into a study abroad program are required to sign this document digitally as part of the online application process.
Transcript Request Form This form is required only for non-UConn students. It provides UConn with the information necessary to send a final transcript to your home institution after you complete the program.
